On the first visit:
A patient completes a brief questionnaire for a permanent record. Each patient brings their health insurance card with all the insurance information, a photo ID and a list of all medications. Each patient advises us of any allergies or health concerns.
A parent or legal guardian attends the examination of the minor under their care.
